Kindergarten Entrance Requirements

In Minnesota, all children entering kindergarten must be five years old on or before September 1st. St. Joseph's School agrees with, and follows this policy.    

Early childhood screening is mandatory for all in-coming Kindergarten students and is offered free of charge in the district where you live.  State law requires that parents/guardians provide verification of early childhood screening for children entering Kindergarten.  If you have not already done this screening, you need to contact the school district where you live and schedule an appointment  (note: most screening locations are closed during the summer).  

Families living within District 197

• If you live within District 197, St. Joseph’s School will obtain a copy of your child’s screening from the 197 Early Learning Office.  You do not need to send any paperwork to the school.

• If you live outside of District 197, you will either need to supply our school office with a copy of your child’s completed screening, or contact our office (651-457-8550) and provide the location of your screening prior to August 1^st. 

Heath forms, including physical examination forms and immunization records must be brought or mailed to the school health office prior to the first day of school.  These forms can be printed using the link below.

1. Physical Examination - to be completed by your child’s health care provider
2. Immunization Record – to be completed by parent/guardian or your health care provider
3. New student health form – to be completed by parent/guardian
4. Consent to share health data – to be completed by parent/guardian
